JOB SUMMARY

Summarized below are the purposes and the main elements of this position: This position reports Cardiology Director. The position includes monitoring clinical effectiveness based on knowledge of best practice standards and adheres to regulations set by regulatory agencies. This position will work interdepartmentally under the direction of the Clinical Director to achieve optimal patient outcomes. 1. Provides leadership and direction in collaboration with the Clinical Director, for the Heart Failure program in accordance with the organizations mission and values. 2. Collaborates with other disciplines to achieve overall Health System and Heart Failure program goals. 3. Under the guidance of the Clinical Director, the Heart Failure program director will help in the development, implementation and evaluation of policies, programs and services that are evidence-based and consistent with professional standards and values. 4. Is accountable for working with the Clinical Director on measurement, assessment, and improvement of the Heart Failure program and patient processes and organizational outcomes. Utilizing data, the PDSA format and meeting targeted goals for the Heart Failure patient and program. Will commit to working and promoting the Cardiology and Health system structure. 5. Ensures a professional nursing practice environment that empowers nurses and provides effective, safe, compassionate, and quality care as evidence by ongoing support of magnet designation and supports the Nursing governance structure and leadership. 6. Is accountable to collaborate with Clinical Director to ensure associates and providers, and all health care providers receive mandatory and annual education to ensure the competence and on-going professional growth of assigned associates. 7. Works with the Clinical Director to pursues and maintain nationally recognized program accreditations, certifications, and designations. 8. As Heart Failure coordinator, in collaboration with the administrative leadership, will implement and maintain the program and provide overall direction and monitoring of patient outcomes and core measures for the heart failure patient. Responsible for implementing program changes to ensure program success as patients or physician needs change and/or any regulations associated with the program change. 10. Assist in making sure that community outreach and patient education events related to the early detection and prevention of heart disease are met, with a minimum of quarterly participation. 11. Implements daily practices to support the patients plan of care with caregivers and providers. 12. Serves as a connection between the public and the facility. 13. Supports and provides transition of care best practices including but not limited to referrals, follow up appointments and calls, including patient and family education. 14. Supports a shared decision making model in all aspects of care.

MAJOR JOB RESPONSIBILITIES

Position areas of responsibility to support ongoing growth and development.

Accept organizational responsibility for the promotion of heart failure services through community outreach.

Maintain and evaluate systems to facilitate planning for delivery of care. Coordinate nursing care with other healthcare discipline and assist in integrating services across the continuum of healthcare.

Evaluate the plan for provision of patient care and attainment of defined outcomes.

Evaluates and monitors quality and nurse sensitive indicators for appropriateness of care for assigned areas of responsibility.

Ensure appropriate orientation, education, credentialing and continuing professional development for associates in collaboration with the Regional Director for Cardiology. Works collaboratively with the NETX Regional CV team.

Works collaboratively with the Clinical Director, physicians, and associates to meet quality and program management initiatives.

Seeks opportunities for educational growth to promote new, innovative and current best practices for Heart Failure.

Understands that Health Care is everchanging and sometimes will need to aide administration or the Clinical Director in other leadership duties, such as survey readiness, patient care initiatives, magnet or nursing activities, and outreach opportunities.

JOB STANDARD QUALIFICATIONS

A. Education/Skills

  • Minimum of associate degree in nursing or a relevant clinical discipline. Language Ability: Excellent communication skills. Reasoning Ability: Ability to use critical thinking Computer Skills: Must be proficient in use of Microsoft Excel, Word, PowerPoint, and the Internet.
  • Bachelors Degree in Nursing is preferred
  • Associates Degree in Nursing required

B. Experience

  • Minimum of five (5) years of experience in a strategic quality position of a complex health care environment.

C. Licenses, Registrations, or Certifications

  • Current RN Texas license is required
